Role Purpose To support the effective delivery of community based palliative care services by coordinating community outreach, patient registration, follow-up visits, and linkages between patients, caregivers, and the multidisciplinary palliative care team. Key Responsibilities · Act as the primary community link between patients, caregivers, and the palliative care team. · Identify volunteers. · Identify and register eligible palliative care patients in the community in coordination with ASHAs, ANMs, CHOs, and health facilities. Coordinate and schedule home visits for the palliative care physician, nurse, and physiotherapist. Support patient counselling, caregiver orientation, and awareness on available palliative care services. Facilitate referrals to hospitals, government schemes, and social support services as required. Assist in organising community meetings, awareness sessions, and outreach activities. Strengthen the stakeholder relationship. Maintain patient records, visit logs, consent forms, and basic MIS data. Track follow-ups, missed visits, and patient outcomes and report to the programme team. Support documentation for monthly reports, donor reporting, and programme reviews. Ensure respectful, ethical engagement and confidentiality of patient information. ​ Requirements Qualifications & Experience Graduate in Social Work / Sociology / Psychology / Public Health or related field preferred Minimum 1–2 years’ experience in community health, palliative care, or NGO field work Experience working with elderly, chronically ill patients, or home-based care programmes desirable Skills & Competencies Strong community mobilisation and coordination skills Empathetic and respectful approach towards patients and families Good communication and basic counselling skills Knowledge of Local Language is must Ability to maintain records and simple reports Willingness to travel locally and work in community settings Disclaimer: The Job Description above is not an exhaustive list of skills, efforts, duties, and responsibilities associated with the position.
