Position Summary- Position is a combination of in-home supports as well as Community Access in Spirit River. Morning shifts 8am start, days to be determined.
Reporting to the Team Leader, the Community Disability Services Worker is responsible for providing support to Individuals in accordance with Blue Heron’s philosophy, the Principles of Support and the Individual’s Personal Support plan. The Community Disability Services Worker provides support that facilitates the Individual’s growth, development and achievement of personal goals.
The Community Disability Services Worker works closely and in consultation with the Team Leader, Program Coordinator and the Individual’s support team.
1. Specific Duties and Responsibilities

a. Individual Support

• Encourage community involvement for each Individual, providing opportunities for structured and informal involvement.

2. QUALIFICATIONS

a. Education

• High School Diploma
• Foundations or Basic Skills Training
• Medication administration

or

• Relevant Certificate or Diploma in Human Services

SIX MONTH’S EXPERIENCE IN THE HUMAN SERVICES FIELD PREFERRED.
