Description The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y. Jordan Health is now seeking a Community Engagement Program Coordinator who, under the guidance of the Chief of Communication and Engagement Officer, is primarily responsible for advancing Jordan Health’s presence in focused patient communities through outreach activities and community-based organization partnership. Requirements The Community Engagement Program Coordinator will ensure that: · Awareness of Jordan Health priorities is broadened and the visibility of Jordan Health programs within targeted audiences is increased. · Diversity of the services most used by (the focused community group) coming to Jordan Health for health care are increased. · Community engagement programs and activities are cost effective and demonstrate full support of Jordan Health’s strategic community engagement goals. Education And Experience Required: · High School diploma and post-secondary education; Associates preferred. · 3-5 years of community organizing and demonstrated community engagement/outreach experience or equivalent combination of education and experience. · Demonstrated ability to relate and communicate with diverse populations and groups. · Effective computer/internet skills · Knowledge and understanding of the Microsoft Office suite of software, specifically Excel, Word, and PowerPoint. Certifications And Licenses · BLS through the American Heart Association · NYS Infection Control Certificate · Valid Driver license and reliable, available transportation Special Skills, Knowledge Required: · Demonstrate experience, ideally in the community health arena, in: 1. Community-based organization partnership development; 2. Community meeting facilitation; 3. Event planning and promotion; 4.
