Description Lifeworks is seeking a full-time, exempt CFSS Service Coordinator to provide case management for clients and families enrolled in Financial Management Services. This role supports individuals with disabilities by managing fiscal budgets, ensuring funds are used to meet personal needs and life priorities, and maintaining expertise in Minnesota waiver programs and related regulations. As part of the Self-Directed Services team, the CFSS Service Coordinator enjoys significant autonomy while managing their caseload and delivering high-quality, person-centered support. Are able to work independently; organize work consistently and meet deadlines. Demonstrate skills in problem-solving with an optimistic and compassionate nature. Collaborate with co-workers across departments and teams. Must pass a background study with the Department of Human Services, a drug test, clean driving record and have reliable transportation to use for work driving purposes (e.g., in-office, on-site training locations, etc.). How you'll be spending your time: Managing a caseload of clients and overseeing individual fiscal budgets by applying strong mathematical skills, leveraging technical tools, and utilizing spreadsheets for accurate financial tracking. Conducting intake meetings with individuals and their support teams to review and finalize Service Plans. Empowering individuals with disabilities and their support networks to lead self-directed lives by effectively utilizing available funding to meet personal needs and priorities. Maintaining in-depth knowledge of Minnesota waiver programs and relevant Minnesota Department of Human Services (DHS) regulations to ensure compliance and quality service delivery. Communicating effectively verbally, in writing, and in person with individuals receiving services, their support networks, Lead Agencies, and referral sources. Providing education and guidance to individuals and their support systems to enhance understanding and successful implementation of self-directed services. Requirements High School diploma with a minimum of three years direct care experience with disabilities or those aged 65 older or in a related human services field. Bachelor’s or Associate's degree in Human Services or related filed with 2 years of relevant work experience preferred. Demonstrated interpersonal skills that quickly build rapport with customers, Lead Agencies, and administrative staff. Case management experience related to self-directed services preferred.
