Description

Support will be provided to a high school-aged teen at her home in rural Rensselaer County, NY, and in various community settings.

QUALIFICATIONS

  • Valid NYS Driver’s License and reliable transportation required.
  • Must be responsible, patient, and supportive, with good judgment in supervision.
  • Experience working with individuals with developmental disabilities is preferred but not required—training will be provided.
  • Ability to work cooperatively with the family and follow care plans.

We are seeking a compassionate and reliable Community Habilitation (ComHab) Worker to support a friendly and active teen who is enrolled in OPWDD Self-Direction services. She is nonverbal, communicates through gestures and a communication device, and benefits from patient guidance, structured activities, and support with daily living skills. The ComHab Worker will:

  • Provide one-on-one support to help her increase independence, social skills, and community engagement.
  • Supervise her at home and during outings, ensuring safety at all times.
  • Assist with daily living skills, including personal care and toileting as needed.
  • Encourage participation in recreational activities such as music, camping, hiking, and other enrichment programs.
  • Support her in exploring her community safely and meaningfully.
  • Follow established support strategies to teach skills for independence, safety awareness, and positive social interaction.
  • Provide transportation to and from activities (mileage reimbursement available)
