JOB OVERVIEW
We are seeking to appoint an enthusiastic, motivated and reliable Community Health Care Support Worker, who is keen to deliver a range of quality care services to patients visited by the Community Health Support Team based at Civic Way, Ellesmere Port. The team visits patients and their carers of all the Integrated Community Care Teams, across Western Cheshire.

As part of a friendly team, you should have experience of working in patients’ homes, the ability to carry out tasks such as simple dressings, catheter care, personal care, recording blood sugars and have experience of providing care for palliative patients. This post involves Lone working, providing overnight care to those in the final days/weeks of life to patients in their own homes.
The hours are 27 per week. Consisting of 3 x 9 hour shifts, worked on a rota basis between 22.00 – 07.00 including Bank holidays and weekends.

The Trust currently supports the Real Living Wage (RLW) hourly rate as set by the Living Wage Federation and will apply a pay supplement if the national A4C hourly rate falls below this.
