We are seeking a Community Health Nurse, pediatric experience is a plus, to provide a variety of professional population-focused community and public health nursing services in health promotion, prevention and disease control programs; and to perform related duties as assigned.

COMMUNITY HEALTH NURSE 1: TRAINING & EXPERIENCE:

  • Associates degree in Nursing from an accredited college or university is required. College-level coursework in the social sciences, and additional nursing training within a community or public health curriculum desirable.
  • Nursing experience in public health or community health care services desirable.

COMMUNITY HEALTH NURSE 2: TRAINING & EXPERIENCE:

  • Bachelor of Science in Nursing degree from an accredited college or university.
  • Or any equivalent combination of experience and training that provides the required knowledge and abilities is qualifying.

SPECIAL REQUIREMENTS:

  • State of Oregon Nursing License- (copy of original or web document)
  • Diploma (copy of original is fine)
  • NPI (downloaded copy acceptable) NPI should reflect Oregon address by time of hire
  • Current BLS/CPR (copy of original is fine)
  • Valid Photo ID must be presented at the Letter of Hire meeting

Responsibilities
  • Performs independent, comprehensive telephone and clinical triaging, and facilitates care coordination through a multidisciplinary approach.
  • Provides patient education as an integral part of the services offered.
  • Acts as relief nurse, triages and screens walk-in patients to assess the severity of the patient’s condition and the need for provider intervention.
  • Communicates with providers regarding determinations and situations, documents events appropriately.
  • Prepares and administers oral, subcutaneous, intra-muscular and intravenous medications.
  • Independently instructs and demonstrates to patients how to provide self-care.
  • Instructs patient in self-medication of prescribed pharmaceutical agents.
