QUALIFICATIONS & SKILLS

  • Knowledge of local healthcare resources and services
  • 2+ years of experience in community health outreach or a similar role
  • Excellent interpersonal and communication skills
  • Ability to work with diverse populations and build relationships with community members. Compassionate and knowledgeable about the community.
  • Strong organizational and time management skills
  • Ability to work independently and as part of a team

Responsibilities
  • Conduct outreach and education activities to increase awareness of Saint Mary’s health resources and services
  • Attend Health fairs, community events, Local Churches events, senior buildings and tabling events by community centers and local businesses.
  • Build relationships with community members, healthcare providers, and local organizations. Attend local meetings and share information on Saint Mary’s and our services.
  • Assist individuals in accessing healthcare services and resources. Recruit referrals with social workers and community leaders.
  • Collect and analyze data to identify health needs and trends in the community
  • Attend weekly staff meetings with the Nursing Director. Submit weekly report on outcomes to the Nursing Director.
  • Develop and implement community-based health initiatives to address identified health needs.
  • Design a monthly calendar outline of activities and potential clients.
  • Provide ongoing support and follow-up on potential clients and share feedback with Director of Nursing.
