POSITION SUMMARY

The Community Health Program Manager oversees all aspects of the Community Health Program. This includes functioning as a direct care provider, program administrator, liaison between PatientCare and the health system(s), business client, covered members, and PatientCare as well as the PatientCare Community Health Medical Director. This position will manage all aspects of client data captureand reporting.

MINIMUM QUALIFICATIONS

  • Valid Florida Drivers license and meet Company driver requirements
  • Possess and maintain current certifications in the following:
    o Florida State Registered Nursing License, or
    o Florida State Paramedic License
    o Cardiopulmonary Resuscitation Provider (CPR)

o Possess and maintain current certifications in Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS).

  • Demonstrated ability to develop and maintain effectiveworking relationships.
  • Demonstrated ability to work in a fast-paced environment.
  • Working knowledge of Microsoft Windows programs,including ability to use spreadsheets and create graphs.

PREFERRED QUALIFICATIONS:

  • Geriatric Emergency Medical Services Training (GEMS)
  • 3-5 Years of Previous Community Paramedic or Home Healthcare Experience

Responsibilities
  • Enter all clients into the 3rd Party Application,
  • Ensure that all client data and client appointments are entered into the Computer Aided Dispatch (CAD) system,
  • Evaluates the residential living environment for safety, environmental, and nutritional considerations and risks.
  • Conduct Program Team Meetings on a regular schedule to ensure that any changes to clients care plans, equipment and supply needs, reporting needs, and training needs are being addressed,
  • Develop a marketing plan to engage potential new clients,
  • Conduct audits on all client interactions for the initial 90-days of the program,
  • Review all protocols and provide program updates as needed with Medical Director approvals,
  • Provide noteworthy updates to the Executive Team at weekly Director’s meetings,
  • Provide audits, updates, and reports as required by contractual obligations for customers,
  • Work with the Communications Center to update call-intake and follow-up call processing scripts,
  • Provide training and educational support for Communications Center Staff and all Program Community Paramedics,
  • Policy and procedure updates
  • Call handling and subscriber disposition updates
  • Review of KPIs against goals
  • Forecast KPIs and expectations
  • Community Health Program Managers undergo about 60 hours of paid training as part of their orientation process.
  • Performs other duties as assigned by management.

  • Judgment/Decision Making – Evaluates the best method of research and then exercises appropriate judgment in establishing priorities and resolving complex matters. Considers the relative costs and benefits of potential actions to choose the most appropriate one.
  • Communication – Ability to communicate ideas effectively, including the preparation of reports and logs. Ability to listen and understand directions, information and ideas presented verbally and in writing.
  • Time Management – Ability to plan and organize daily work routine. Establishes priorities for the completion of work in accordance with sound time-management methodology.
  • Ability to successfully pass a thorough background investigation and drug test
