JOB SUMMARY

The Community Health Worker (CHW) at 20/20 Vision Youth serves as a trusted frontline public health advocate and resource navigator for vulnerable children, youth, and families in the Inland Empire. CHWs connect clients to critical health and social services, reduce access barriers, and support individualized care plans as part of a trauma-informed, culturally responsive team aligned with the Community Health Worker (CHW) model.
This role involves care coordination, advocacy, outreach, case management, data tracking, and service delivery in both community and virtual settings.

HEALTH EDUCATION & OUTREACH

• Provide health education that aligns with cultural, linguistic, and community needs
• Promote healthy behaviors and lifestyle changes through coaching and awareness activities
• Conduct community outreach to identify at-risk individuals and educate families about available resources

REQUIRED QUALIFICATIONS

• High school diploma or equivalent; associate or bachelor’s degree in human services, public health, or related field preferred
• 1+ year experience in community outreach, peer support, case management, or related role
• Strong communication skills and ability to relate to diverse populations
• Familiarity with Medi-Cal, IEHP, CHW services, and/or CalAIM programs
• Computer literacy (Google Workspace, Google Drive, fillable PDFs)
• Ability to maintain accurate records and comply with confidentiality requirements (HIPAA)
• Bilingual in English and Spanish (preferred)
