Responsibilities

Under supervision of the Community Health Director, this innovative, grant funded 12/31/2023 Community Health Navigator(CHN) position will help assist patients admitted to the hospital with alcohol and/or drug dependence with their insurance and resource needs in preparation for and post discharge. The CHN serves as an advocate for patients, providing support and education so they are better able to engage in their care. This position offers a hybrid work opportunity, with some travel between hospitals for onsite responsibilities coupled with working remotely.
