Description

PHYSICAL REQUIREMENTS

This position requires travel primarily in a 30 mile radius of the office. It also requires extensive documentation in an electronic health record. Lifting requirements are approximately 15 pounds and the ability to provide physical support to some clients on a non-routine basis.

REQUIREMENTS

Meets at least one of the following:
1. Meets the criteria to bill as a community support.
2. Has a bachelor’s degree in social work, psychology, or closely related fields or is a licensed nurse.
3. Equivalent experience may be substituted on the basis of one year of experience for each year of required education.

REQUIREMENTS

Class E Driver’s License and a good driving record.
Certifications: BLS, Nonviolent Crisis Intervention, Level 1 Medication Aid.

REQUIREMENTS

At least one year in case management with adults and/or youth with serious and persistent mental illness or substance use disorders

Responsibilities

RESPONSIBILITIES

  • Use PRAPARE tool to assess Social Determinates of Health (SDOH) issues that are potentially impacting health care status.
  • Engages persons in a productive working relationship through to address SDOH and provide needed support to the patient.
  • Develop a treatment plan and document progress toward the goals of the treatment plan
  • Review progress with the treatment plan on a quarterly basis and make course corrections as needed with the treatment team.
  • Engages natural supports in the treatment of persons served in the program
  • Assist clients in effectively using available resources both in the center and the community by assisting with scheduling appointments, tracking the status of referral completion.
  • Provides timely, accurate and quality documentation.
  • Manages time well as evidenced by being on time for appointments, attending staff team meetings, and completes documentation on time.
  • Works to improve the overall health status of individuals served.
  • Able to prioritize workload.
  • Works as part of an integrated team.
  • Work with the Quality Improvement Team and provide support for quality improvement PDSA projects.
  • Must be able to sit and drive for a significant part of a work day.
