Description Community Health Worker Compensation (Based on experience and qualifications): Minimum $18.34 Schedule: Monday - Friday 9:00am - 5:00pm Under the supervision of the Clinic Manager, the Community Health Worker (CHW) will work independently in the community, focusing primarily on individuals affected by HIV. The CHW will collaborate with healthcare providers and social service agencies to connect patients with resources that improve care and health outcomes. Acting as an advocate for CFHC patients living with HIV, the CHW will develop and implement targeted outreach initiatives to promote the health and well-being of these patients and their families. Responsibilities: Promote and communicate CFHC programs and services to patients and targeted community groups, with emphasis on individuals affected by HIV. Serve as a liaison among patients, healthcare providers, and community resources to improve access to culturally appropriate health and social services for those impacted by HIV. Identify, engage, and support high-risk and underserved populations living with HIV, addressing cultural and socioeconomic barriers to care. Provide education and coaching on health promotion, chronic disease management, self-care, and appropriate healthcare utilization, including alternatives to emergency room visits. Coordinate patient care activities such as referrals, follow-ups, transportation, eligibility assessments, and connections to medical homes and social services tailored to HIV patients. Assist patients in understanding care plans, treatment protocols, and health goals, ensuring ongoing follow-up and support. Build and maintain collaborative relationships with patients, providers, clinic staff, and community partners using a multidisciplinary team approach. Document patient records, service plans, and related information accurately and confidentially in accordance with organizational standards. Maintain high standards of customer service and patient confidentiality in compliance with HIPAA and all relevant legal and ethical guidelines. Bilingual ability is preferred but not required. Knowledge/Skills/Abilities Demonstrated ability to collaborate effectively in a team setting. Ability to maintain effective and professional relationships with patients and other members of the care team. Experience in working with a diverse population base preferred. Ability to appropriately handle confrontation and conflict. Ability to work independently and to seek consultation as necessary. Ability to follow organization and departmental processes and procedures. Ability to work in a high stress/fast paced/high work volume environment.
