QUALIFICATIONS

  • Experience in community health, nursing, social work, case management, or public health (preferred).
  • Knowledge of healthcare systems, including Medi-Cal/Medicare, and comfort with medical terminology.
  • Experience working with people with disabilities or high-need populations strongly preferred.
  • Strong communication skills, including teaching, public speaking, and motivational interviewing.
  • Understanding of nutrition, addiction recovery, or health promotion (a plus).
  • Ability to work independently and collaboratively within a multidisciplinary care team.
  • Excellent interpersonal skills with a genuine commitment to serving the community.
Responsibilities
  • Conduct outreach to engage and build trust with community members.
  • Provide education on health and wellness topics, including nutrition, preventive care, and chronic condition management.
  • Assist clients in navigating healthcare systems, including Medi-Cal, Medicare, and social service programs.
  • Use motivational interviewing to support clients in making positive, informed health decisions.
  • Collaborate with healthcare professionals to develop and implement personalized care plans.
  • Advocate for individuals with disabilities to ensure equitable access to care and services.
  • Facilitate workshops, classes, and small group sessions on health-related topics.
  • Collect and track data to assess community health needs and program outcomes.
  • Maintain accurate, timely documentation of client interactions and progress.
