POSITION REQUIREMENTS – EDUCATION AND EXPERIENCE

• High school diploma or equivalent required.
• Bilingual in Spanish preferred, but not required.
• Familiarity with the community being served.
• Strong communication skills and a customer service mindset.
• Team-oriented with the ability to work independently and collaboratively.
• Experience working with underserved, high-risk, or disadvantaged populations.
• Basic computer proficiency.
• General knowledge of medical terminology and health literacy.
• Ability to build and maintain positive relationships with care teams and partner organizations.
• Willingness and ability to provide emotional support, encouragement, and motivation to patients.
