New inits for pedi/family support Job Summary Summary Responsible for working closely with individuals and communities to promote and maintain good health. Responsible for providing education, support, and outreach services to community members to help them achieve their health goals. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors. Works under the direction of a healthcare professional to provide preventive care and health education to members of the community. Does this position require Patient Care? Yes Essential Functions -Conduct community outreach and engage community members to understand their health needs -Provide individual and group health education and coaching to community members -Assist community members in accessing and navigating the healthcare system, including providing referrals to appropriate health services -Provide basic health screenings and assessments to identify health risks and provide appropriate interventions -Assist in the development of health promotion programs and activities to address community health needs -Collect data and maintain accurate records of community health outreach activities and interventions -Attend meetings and trainings related to community health promotion and education Qualifications Education High School Diploma or Equivalent required or Associate's Degree Social Work preferred or Bachelor's Degree Related Field of Study preferred Can this role accept experience in lieu of a degree?
