Job Summary: The Early Childhood Community Health Worker serves in the classroom as a teacher assistant to meet the needs of families of the program and assist the teacher to implement an excellent early childhood kindergarten preparatory program. The Early Childhood Community Health Worker will work to build relationships with families and children to provide access to the health center. Through that relationship, the CHW will serve as a liaison between families and resources. As an Early Childhood Community Health Worker, the goal is to facilitate access to care, improve quality of life and address the effects of social determinants of health in addition to assisting in the educational development of each child. Key Responsibilities: Implement and assist with daily lesson plans. Maintain daily class schedule. Ensure daily communication is maintained with parents through communication logs, calendars, newsletters, office notes. Daily connection with students Daily communication with on site manager Daily record keeping Daily maintenance of classrooms Attend staff meetings and ongoing trainings. Assist in and attend program events. Works directly with on site manager to identify needs and developmental goals for each student. Submits needs to manager as necessary. Works directly with on site manager to implement family education and support. Work to build the overall development of the child through engaging instruction and interaction using the resources provided. Work with families to assist in meeting the needs in the area of medical, dental and social through monthly meetings and events. All other duties as assigned. Requirements Required Qualifications: Valid Driver’s License. Pediatric First-Aid, CPR and AED certification required within 90 days of hire.
