Position: Community Health Worker
Location: East End Community Health Centre, 1619 Queen Street East
Position type: 0.7 FTE (26.25 hours per week)
Start Date: Open until filled, preferably October 1, 2025
Hours of work: Flexible, including 1 evening/week and 1 Saturday/month

POSITION SUMMARY

East End Community Health Centre is a non-profit, multi-service centre providing health, social and community outreach services. We provide comprehensive health services in collaboration with clients and community partners to promote health and well-being in the East End of Toronto. Our approach aims to improve inclusion, equity and access, with a focus on those experiencing barriers to care.
As a member of the interdisciplinary team, the Community Health Worker works within the framework of the social determinants of health to provide client-centered care that emphasizes both health promotion and disease prevention. The Community Health Worker leads, assists and/or supports action on a range of community health issues through various health promotion groups, client support, community outreach, and community engagement activities.
The Community Health Worker is expected to participate in quality improvement initiatives, execution of strategic plans, and implementation of organizational accountability systems.
The successful candidate will work in partnership with other staff and management to support the Vision, Mission and Values of the Centre. They will be supervised by the Director of Allied Services and Community Engagement.

QUALIFICATIONS

  • Strong experience with supportive listening, life coaching and providing emotional support is essential
  • An undergraduate degree in a health or social science discipline from a recognized university or significant related work experience
  • A minimum of five years of relevant experience in a community setting
  • Ability and commitment to work within the Centre’s vision, mission, strategic priorities and CHC Model of Care
  • Willingness to work with priority populations and within organization’s anti-discrimination framework.
  • Experience in health promotion program development, implementation, monitoring and evaluation; previous experience in running health and wellness groups an asset
  • Proficiency in assessment, client support services, referrals and follow-up
  • Excellent interpersonal skills with clients in a culturally diverse practice population, and with colleagues
  • Familiarity with electronic client records and computer literacy
  • Excellent written and verbal skills
  • Excellent communication, decision-making, problem-solving, conflict management and time management skills.
  • Strong knowledge of health equity indicators and social determinants of health facing vulnerable and marginalized individuals
  • Second language, relevant to catchment area populations, is an asset.
  • Must be vaccinated against COVID-19

Responsibilities
  • Offer supportive listening, life coaching and emotional support
  • Perform comprehensive client intake assessments, including financial needs
  • Help clients meet their basic needs for: shelter, safety, health cards, immigration, and social assistance by providing information and support, and facilitating access to support services and resources as appropriate
  • Inform and support development, implementation, and evaluation, of community health groups/activities as needed
  • Obtain, document and interpret client information and maintain client records according to the standard of medical records at the Centre including assuring privacy and confidentiality
  • Actively promote participation in Centre programs and services through ongoing outreach to target populations
  • Identify, build, and maintain partnerships with community agencies to enhance service delivery and build capacity
  • If funding allows, plan and provide nutritious snacks/meals to group participants as appropriate
  • Assist in the development and monitoring of project budgets; consistently follow organizational guidelines on resource spending and approval
  • Set-up, clean-up, and put away resources used to provide programs
  • Participate in quality improvement initiatives such as group client satisfaction surveys and chart audits
  • Plan, organize and coordinate one to two of our Centre groups regarding health and wellness—could be on site or in the community
  • Supervise students and volunteers as required
  • Maintain professional competence through appropriate continuing education
  • Comply with all relevant Centre policies, procedures, and protocols
