POSITION TITLE: Community Health Worker
FLSA STATUS: Non-Exempt, Full-Time
REPORTS TO: Program Manager
LOCATION: Applicants must be local to Santa Clara County and able to work in-office from our San Jose location.
Job Summary: The Community Health Worker (CHW), Education serves as a key support to program facilitators, partner sites, and families to ensure the success of the Education programs. This role includes scheduling internal and external referrals, supporting class logistics, tracking attendance, and teaching classes. In addition, the CHW, Education acts as a trusted community liaison—empowering individuals and families to improve their health and well-being through culturally responsive outreach, resource navigation, and hands-on support. The ideal candidate is empathetic, organized, and community-focused, with a strong commitment to health equity and a willingness to grow with the role.
PROGRAM DELIVERY AND EDUCATION
- Support the facilitation of health education programs, ensuring smooth operations for both in-person and virtual classes
- Prepare, moderate, and support class sessions by coordinating materials, technology, and logistics
- Conduct presentations to children and caregivers on topics such as oral hygiene, healthy habits, wellness, and prevention
- Educate clients and families on health conditions, resources, and preventive care during one-on-one and group sessions
- Utilize evidence-based practices and tools based on organizational priorities and training
- Gather and verify attendance records and outreach data using Salesforce or other systems to ensure reporting accuracy
- Conduct surveys, collect testimonials, and track outcomes to evaluate program impact and inform improvements
- Research and develop new partnership opportunities to support program growth and resource access
- Create content for social media and other platforms to promote health initiatives and program visibility
EDUCATION AND CERTIFICATION:
- High school graduate or GED
- Associate or Bachelor’s degree preferred for CHWs providing ECM services
- Community Health Worker Certificate required, or obtain one within 6 months of start date
EXPERIENCE
- A minimum of 18 months working to support the health or social well-being of marginalized, high risk and underserved populations
- Experience using computer for documentation, communication, and organizing daily tasks, including Microsoft Office and databases
- Lived experience that aligns with and provides a connection between the CHW and the Member or population being served.

- Available for 8-hour shifts at the worksite with start times 7:30am-10:00 am and end times 3:00-7:00 pm and occasional evening or weekend hours as needed
