Sentara Health is hiring for a Community Health Worker in Hampton Roads (Norfolk, Virginia Beach, Newport News)!

REQUIRED QUALIFICATIONS:

  • Driver’s License – Certification – Other/National REQUIRED
  • HS – High School Grad or Equivalent REQUIRED

PREFERRED QUALIFICATIONS:

  • Experience working in a community-based setting preferred.
  • Knowledge of medical terminology preferred

Responsibilities
  • Works in collaboration with the research Manager and other research team members to provide comprehensive and coordinated care.
  • Engages in interventions to improve self-management skills and improve quality of life for patients needing workup for high blood pressure diagnosis.
  • Provides education and behavior modification strategies and provides social support to patients and families.
  • Acts as a patient advocate and healthcare navigator in assisting with resource allocation and management.
