Description We are seeking an experienced Community Resource Specialist who is a certified Community Health Worker (CHW) to join our team on a during the upcoming Open Enrollment period. This role is ideal for someone who can hit the ground running to support patients with insurance applications, sliding-fee programs, and community resource connections. Key Responsibilities Assist patients with insurance and benefits navigation (Marketplace, NH EASY, Medicaid, etc.) Process sliding-fee scale applications, discuss payment options, and arrange payment plans Follow up on applications and patient contacts within established timeframes Provide advocacy and education on available community resources (food, housing, transportation, etc.) Support outreach and community events (some evenings/weekends) Collaborate with billing and patient services teams to resolve coverage or payment issues Verify and update patient demographics and consents during interactions Serve as a knowledgeable point of contact for patients needing assistance during Open Enrollment What We Offer A chance to make a direct impact on community health during a critical time of year Collaborative team environment with training on HealthFirst systems and workflows Requirements Required: Current Community Health Worker (CHW) certification High school diploma or equivalent Minimum 2 years’ experience in a healthcare or insurance benefits setting Strong customer service, communication, and computer skills Preferred: Certified Application Counselor (CAC) for CMS Familiarity with medical terminology and cross-cultural communication
