Description

EDUCATION AND EXPERIENCE:

Possession of a high school diploma or equivalent is required. A valid California driver’s license, reliable car, and valid automobile insurance are also required. Innercare will verify these requirements, as automobile travel within the Inland Empire required.
Certification as a CHW, or at least 2 years of experience as a CHW, Promotora, or Health Navigator, or 2 years’ experience working in community outreach, customer service, or within a medical office, or a Behavioral Health or Substance Use Disorder program.

DESIRED QUALIFICATIONS:

· Bi-lingual English/ Spanish desired
· Knowledge of providing culturally competent services to a variety of diverse patients

KNOWLEDGE AND ABILITIES:

California Laws and regulations for mental health services; non-violent crisis intervention techniques; Evidenced-based practices and philosophy; data collection and record-keeping techniques. Excellent communication skills, good organizational and interpersonal skills and highly motivated. Reading and writing ability in English sufficient for filling out forms, reports, etc. Ability to follow written and oral instructions.
Highly skilled interpersonally, with excellent team work and relationship skills. Highly skilled in interpersonal communication, including resolving conflict. Able to sufficiently engage members in a variety of settings, including on the phone, at Member’s homes, in hospitals and other settings. Knowledge of the community the CHW will be working in, especially non-professional resources, and their reputation in the community. WORKING CONDITIONS: The usual and customary methods of performing the job’s functions require the following physical demands: some lifting, carrying, pushing, and/or pulling; and significant fine finger dexterity. Generally the job requires 85% sitting, 10% walking, and 5% standing. This job is performed in a generally clean and healthy environment. Hearing and speaking to exchange information in person and on the telephone; dexterity of hands and fingers to operate a computer keyboard; seeing to view a computer monitor; lifting up to 50 pounds, carrying, pushing or pulling moderately heavy computer equipment; sitting or standing for extended periods of time; bending at the waist, kneeling or crouching; reaching overhead, above the shoulders and horizontally. Responsibilities
  1. Responsible for assisting Members in navigating the healthcare system by finding and following up by phone and in person with eligible Health Homes Program (HHP) Members in improving their whole health, through outreach and engagement activities, which are primarily field based. This position works collaboratively as part of the Community Based Care Management Entity (CB-CME) team, members and families, and other professionals, in addition to working collaboratively with designated PCP care team.
  2. Model the highest ethical behavior in relationships with co-workers, supervisors, Members, Providers, and colleagues in the community.
  3. Responsible for engaging with Members, both in-person and on the phone, in a manner that utilizes evidence-based approaches (such as Motivational Interviewing) that promotes collaboration between the Member and his or her medical/behavioral team, as well as to increase the Member’s sense of control over their whole health.
  4. Delivering information about health and wellness in ways that the community can easily understand and providing information on HHP Member benefits and services.
  5. Providing advocacy on behalf of HHP Members in the home, the community, and in provider organizations.
  6. Responsible for building and maintaining a positive working relationship with Providers, including, but not limited to, interactions within Provider offices, communication over the phone, and through digital means such as email and fax.
  7. Responsible for assisting the CB-CME and providers to understand the culture, norms, beliefs and preferences of the Members and their community by: representing the voice of community, helping to create messages and materials that fit community culture, and delivering these messages in a way that fits the culture of Members.
  8. Responsible for promoting a collaborative and effective working environment within the CB-CME by engaging in evidence-based communication strategies (such as Motivational Interviewing) when discussing responsibility/sharing of tasks, effectively resolving conflicts as they arise, and collaborating on Member case discussions.
  9. Collaborates on Member care issues with other CB-CME team, participating in weekly and ad hoc case reviews, and consults with Nurse Care Manager and/or Behavioral Health Care Manager before taking any action that is clinical in nature.
  10. Model commitment to continuous quality improvement by engaging in quality improvement initiatives and projects, such as by identifying and addressing gaps, and by identifying, developing, and testing new practices for improving the outcomes of the HHP.
  11. Ensures documentation is accurate, useful and in compliance with regulatory requirements in accreditation standards.
  12. Participates in all appropriate staff meetings or other activities as needed.
  13. Help facilitate interactions with the CB-CME team members.
  14. Ensures the privacy and security of PHI (Protected Health Information) relating to HIPAA compliance.
  15. Performs any other duties as assigned or required to ensure HHP operations are successful.
