Description JOB SUMMARY The Community Health Worker (CHW) supports Enhanced Care Management (ECM) members by improving access to care, promoting whole-person health, and addressing social and behavioral barriers. This field-based role conducts outreach, engagement, and care coordination activities, working closely with the Community-Based Care Management Entity (CB-CME), providers, and interdisciplinary care teams to improve health outcomes. Requirements MINIMUM & PREFERRED QUALIFICATIONS Education/Training Minimum: High school diploma or equivalent Preferred: Community Health Worker (CHW) certification Experience Minimum: 2+ years of community health, outreach, or related experience, or ability to obtain CHW certification Preferred: Experience working with underserved populations or within healthcare/community-based programs Any combination of educational and work experience that would be equivalent to the stated minimum requirements would qualify for consideration of this position. Certification(s) Maintains a valid California Driver’s License and current automobile insurance when using a personal vehicle for Company business CHW Certification (preferred or willingness to obtain) Skills, Knowledge & Abilities · Strong interpersonal and relationship-building skills · Effective verbal and written communication skills · Conflict resolution and problem-solving abilities · Knowledge of community resources and healthcare systems · Ability to engage diverse populations in various settings (home, clinic, phone) · Cultural competency and sensitivity to behavioral health conditions · Ability to work independently and exercise sound judgment · Team-oriented with collaborative working style · Bilingual (English/Spanish) preferred · Commitment to equity, reducing bias, and improving health outcomes PHYSICAL, MENTAL & ENVIRONMENTAL REQUIREMENTS: The physical, mental, and environmental demands described here are representative of those required to successfully perform the essential functions of this job. Primarily field-based role requiring regular travel between Company locations and other business sites as required; including standing, walking, and driving. Involves interacting with members across home, clinic, and community settings, using standard technology, and managing multiple tasks independently in a dynamic environment. PAY RANGE $26.00 - $29.00 / hourly
