Are you looking for an opportunity to advance your career while working with an extraordinary team?  At Merakey, we put heart and soul into everything we do.  We are seeking a Community Health Worker to join our team at our program in our Pittsburgh, PA location supporting Allegheny County.        Earn $21.80/hour.   Position Details    Community Health Workers (CHWs) possess a deep understanding of the community they serve and build trusting relationships with individuals from diverse backgrounds to connect them with resources and care. Their effectiveness stems from their ability to reach underserved and hard-to-reach populations, providing patient support and coaching to achieve healthcare goals by addressing Social Determinants of Health (SDOH) needs. This role involves both face-to-face and telephonic interactions with individuals, families, and the community.   Responsibilities include, but are not limited to: * Complete a standardized Social Determinants of Health (SDOH) assessment for each individual receiving services, initially and at least quarterly throughout the service period. * Provide referrals and link individuals to resources addressing SDOH needs such as housing, transportation, peer support, career training, and social supports. * Identify individuals eligible for Medical Assistance and assist with the completion of forms to enroll or maintain HealthChoices coverage. * Help individuals identify gaps in care, provide linkages to health-related services, and assist in scheduling appointments as needed. * Create connections between vulnerable populations and the healthcare system, promote wellness by encouraging cultural competence among healthcare professionals, and assist with chronic condition management through education and coordination with providers. * Assist individuals in obtaining preventative services by referring them to primary care physicians and advocating for their needs within the healthcare system and community. * Build relationships with local health and social service providers, provide referrals to home and community-based services, and offer schedule flexibility to meet individual needs, including evenings and weekends. * Perform other duties as assigned or requested. Qualifications The ideal candidate will possess the following qualifications:  * High school diploma or GED required. * Associate's degree with focus in psychology, social work, or public health administration preferred. * Certification as a Community Health Worker through the PA Certification Board required, or must be obtained within one year of hire. * Lived experience within the population to be served, which may include aspects such as geography, physical or behavioral health, race, immigration status, SOGIE identification, etc required. * Must have valid driver's license and access to a vehicle. * Knowledge of chronic health conditions preferred.
