Description Position Summary: Community Health Workers (CHW) serve as trusted frontline public health workers who deeply understand the communities they support. Acting as a vital link between individual, healthcare services, and social support systems, CHWs play a key role in: Facilitating access to care, improving delivery service, and enhancing health outcomes. Essential Duties and Responsibilities include the following: Conducts community outreach and assists women and their families in accessing healthcare and social services by offering resources and referrals. Provides case management and assists with care coordination. CHWs provide virtual and/or home visits to support maternal and child health clients. Follows up and assess clients' needs and provides accurate documentation of information in the Unite Us system. Conducts community education sessions and workshops and will maintain a regular presence in the community by participating in health fairs and events. Collaborate with community-based organizations to ensure a comprehensive system of care. Participates and assists with the implementation of quarterly Community Advisory Board meetings. Works with other community-based supportive services/agencies throughout the county to provide a comprehensive system of care to the families served Other duties may be assigned as needed. Working Conditions: There are no unusual work conditions associated with this position. Noise level in the office is moderate. Professional/business attire is required. Employees are required to attend meetings at external locations and will therefore experience external weather conditions. Requirements Education: Minimum of High School Diploma. Bachelor's degree (BA/BS) in Public Health or related field Preferred. Experience: Minimum of 1 year in the maternal child health field. A strong knowledge of their respective counties and the New Jersey social service system and experience in the field of public health, specifically maternal child health, is strongly preferred. Knowledge and experience relating to the field of non-profit. Must be able to speak and write fluently in Spanish and English. Computer proficiency required with Microsoft programs. Strong interpersonal & oral/written communication skills required. A valid Driver’s license with the ability to provide proof of ongoing automobile insurance coverage is required. This position will entail reimbursable local and statewide travel utilizing personal vehicles. Physical Requirements: Employees are required to occasionally lift up to twenty pounds of materials or equipment.
