Position Summary: The Community Health Worker (CHW) supports the Patient Engagement Services program by providing outreach, care coordination, and patient support to underserved and special populations, including individuals experiencing homelessness and immigrant communities. This role works primarily in community-based settings and on the Mobile Medical Team (MMT) to improve access to care, address social drivers of health (SDOH), and facilitate patient engagement in medical and behavioral health services. Key Responsibilities: Outreach & Engagement Conduct community outreach to identify and engage underserved populations, including migrant, immigrant, and unhoused individuals Build trusting relationships with patients through culturally responsive and trauma-informed approaches Provide education on available health services and assist with navigation of the healthcare system Mobile Medical Unit (MMU) Support Support clinical operations on the MMU during outreach events and scheduled visits Assist with patient flow, insurance enrollment, registration, and intake processes Prepare patients for appointments and ensure appropriate follow-up care is arranged Have valid driver’s license and comfort in operating/driving unit for outreach events as needed SDOH Screening & Navigation Conduct Social Drivers of Health (SDOH) screenings (e.g., housing, food security, transportation, employment) Identify patient needs and provide referrals to community-based resources Assist patients in accessing services such as housing support, food programs, transportation, and financial assistance Care Coordination Schedule and coordinate medical, dental, and behavioral health appointments Provide appointment reminders and follow-up with patients to reduce no-show rates Collaborate with interdisciplinary teams including providers, nurses, behavioral health staff, and outreach teams Documentation & Compliance Accurately document outreach activities, patient interactions, and referrals in the electronic health record (EHR) Maintain confidentiality and comply with HIPAA regulations Support data collection and reporting requirements related to outreach and SDOH initiatives Requirements Qualifications: Required: High School Diploma or equivalent Strong connection to or experience working with underserved communities (e.g., migrant, immigrant, or homeless populations) Bilingual (preferred based on community need, e.g., Spanish/English) Excellent interpersonal and communication skills Ability to work flexible hours, including evenings and weekends as needed Preferred: Certification as a Community Health Worker (or willingness to obtain) Experience in outreach, case management, or patient navigation Familiarity with SDOH screenings and community resource networks Experience working in mobile health, community-based, or public health settings Certification as a facilitated enroller or ability to become facilitated enroller Work Environment & Schedule: Saturdays a must, plus two days a week and as-needed based on program and outreach needs Work conducted in community settings, farms, shelters, and on the Mobile Medical Unit (MMU) Requires travel within service areas Core Competencies: Cultural humility and sensitivity Strong organizational and follow-through skills Ability to work independently and as part of a team Problem-solving and resourcefulness in non-traditional settings
Responsibilities
