Description

Empower House provides community health services, youth development programs, and advocacy opportunities that are transformative and empowering for women and girls of color. We envision a world where all women and girls are empowered to transform themselves, their families and their communities.
Empower House is seeking one full-time Community Health Workers (CHW) to implement the Empower Choice initiative. The Empower Choice CHWs will facilitate workshops, peer-support groups, and provide healthcare navigation support to empower women and birthing people with knowledge, information, and resources related to comprehensive reproductive health, including mental health. CHWs will provide education, manage a caseload, conduct home visits, and refer clients to contracted partners and in-house staff to empower community members to choose the reproductive path that is right for them, promoting a greater sense of agency and courage to advocate for their needs. Additionally, CHWs are responsible for timely and accurate participant-level data tracking in our Salesforce database, conducting outreach, and supporting Community Health events and initiatives of Empower House.

QUALIFICATIONS:

  • High school diploma and/or equivalent – bachelor’s degree preferred
  • At least 2 years of non-profit work experience
  • Community Health Worker Certification preferred
  • Excellent interpersonal skills, with the ability to build rapport and act as a trusted advocate within the community we serve
  • Experience with leading and facilitating classes, workshops, and/or peer-support groups
  • Excellent verbal and written communication skills with exceptional attention to detail, supported by strong documentation abilities, proficiency in computer skills, and strong organizational capacity.
  • Skilled in creating, organizing, and presenting information clearly and effectively
  • Experience in community health work, particularly with BIPOC communities, or related fields
  • Understanding of reproductive and mental health within our target population’s socioeconomic and cultural characteristics
  • Bilingual in English and Spanish preferred
  • Must have the ability to thrive in a flexible, fast paced growth-oriented environment
  • Flexibility to work evenings and weekends as needed to meet community needs.

Responsibilities
  • Deliver evidence-based workshops on reproductive health for women, youth, and birthing people; topics include family planning & contraception, STD awareness, healthy pregnancy practices and other reproductive health related topics
  • Conduct four (4) one-on-one consultations with each participant in the Empower Choice program to assist in navigating holistic perinatal services
  • Facilitate group classes and peer support circles for pregnant individuals and new parents
  • Conduct home visits with participants in the Empower Choice Program, as needed
  • Coordinate with the Community Engagement Coordinator, Manager, Volunteers and other EHSA staff to conduct outreach with a focus on BIPOC (Black/African American, Indigenous, People of Color) populations
  • Support participants in accessing a supportive OB/GYN and other perinatal service providers, such as doulas, midwives, and mental health providers.
  • Provide benefits orientation, navigation and enrollment support to community members across a range of public assistance and health coverage programs.
  • Provide direct assistance, as needed, and connect participants to community services within partner agencies as well as in-house
  • Provide culturally appropriate service, acting as a liaison, advocate, and mentor to empower participants to make informed choices about their reproductive health
  • Address barriers to care such as cost, transportation, and mistrust in the healthcare system
  • Connect clients with transportation resources, as needed
  • Collaborate with Empower House Data Manager to track program efficacy
  • Data collection and entry using the Salesforce system in real time to add notes for all participants, including client conversations, meetings, issues, challenges, or additional support needed
  • Use digital tools and platforms (e.g., JotForm, Canva) to support program documentation, communication, and participant engagement.
  • Assist clients in completing applications and registration forms
  • Exhibit supportive and collaborative working relations with community members, visitors and staff
  • Other related duties as assigned by the Community Health Manager
