ESSENTIAL FUNCTIONS:

• Meet patients recently discharged in their home and conduct needs assessment using Social Determinants of Health Tool.
• Schedule weekly follow-up calls and home visits to patients.
• Help patients with social issues like homelessness, substance abuse, hunger, transportation, and many other social issues that are impacting the members lives.
• Assist patients with organizing their records, making follow-up appointments with a Primary Care Physicians or Specialists.
• Help patients with the tasks of getting medical care, and work on health goals by helping to plan or prepare healthy meals or finding time to exercise.
• Guide patients with grooming , dressing, taking medications, and housekeeping,
• Help patient s access resources, including quality of care, childcare services, food, housing, transportation.
• Maintain relationship with the Community Based Organization.
• Able to travel and meet the members where they are; may include local Churches, homeless shelter, and members residence.
• Must have an empathetic attitude toward the care of the sick and elderly to deliver services in accordance with our mission and vison.
