Description LOCATION: New Hope Missionary Baptist Church - Wayne, MI 48184 POSITION SUMMARY: SEMHA is seeking a compassionate and motivated Community Health Worker (CHW) to join the Wayne FQHC Program located in the city of WAYNE. The Community Health Worker is a part of an inter-disciplinary clinical care team at a health center who helps increase access to health services for people through outreach, encouraging engagement, education, and peer support. REQUIREMENTS: Conduct patient outreach and provide education on preventive screenings and chronic disease management. Address HEDIS care gaps and engage high emergency department utilizers to connect with their PCP. Perform Social Determinants of Health (SDOH) screenings and link patients to appropriate community resources. Coordinate referrals, assist with scheduling, and help remove barriers to care. Maintain accurate documentation, outreach tracking, and monthly reporting for Health Plans. Participate in community outreach events and support site visit preparation (e.g., HRSA, AAAHC). Maintain HIPAA compliance and confidentiality at all times. Support patient panel management and other duties as assigned. EDUCATION & QUALIFICATIONS: High school diploma required. Bachelor’s degree in health sciences or related field preferred. MICHW certification preferred. Experience in community work, education, or healthcare strongly preferred. Strong communication, organizational, and interpersonal skills. Ability to multitask in a fast-paced environment and advocate for patients. Proficiency in Microsoft Office preferred. Valid driver’s license and reliable transportation required.
