I. BASIC FUNCTION:
Provides leadership to community health workers to promote Family Care Health Center’s (FCHC) community health programs for patients and their families in the Center, community, and in the home setting. Facilitates access to services and works to improve the quality and cultural competence of service delivery.
All employees of FCHC must ensure service standards are delivered, including:

- Provides timely response to requests, tasks, and inquiries. Demonstrates good service turnaround.
- Demonstrates good communication skills and communicates in a tactful manner.
- Exhibits conflict resolution skills in order to foster effective working relationships and embraces a team
approach.
- Adheres to FCHC’s dress code policies. Employee appearance and grooming appropriate.

VI. QUALIFICATIONS:
- Bachelor’s Degree in Human Services or related field required.
- Community Health Worker certificate and credentialing required.
- Minimum of 5 years of work experience in a community health care setting required.
- Excellent verbal and written communication skills required.
- Must be proficient in Microsoft Excel.
- Desirable experience includes customer service, patient/peer counseling, medical patient interaction, community relations, outreach, data entry, public speaking and non-profits.
VII. CONTINUING EDUCATION:
- Active professional development as demonstrated through attendance in seminars, continued education programs, etc. is strongly encouraged.
- Satisfactory maintenance of continuing education standards as established by the Center and regional commission for Community Health Workers is required.
