I. BASIC FUNCTION:

Provides leadership to community health workers to promote Family Care Health Center’s (FCHC) community health programs for patients and their families in the Center, community, and in the home setting. Facilitates access to services and works to improve the quality and cultural competence of service delivery.

VI. QUALIFICATIONS:

  1. Bachelor’s Degree in Human Services or related field required.
  2. Community Health Worker certificate and credentialing required.
  3. Minimum of 5 years of work experience in a community health care setting required.
  4. Excellent verbal and written communication skills required.
  5. Must be proficient in Microsoft Excel.
  6. Desirable experience includes customer service, patient/peer counseling, medical patient interaction, community relations, outreach, data entry, public speaking and non-profits.

VII. CONTINUING EDUCATION:

  1. Active professional development as demonstrated through attendance in seminars, continued education programs, etc. is strongly encouraged.
  2. Satisfactory maintenance of continuing education standards as established by the Center and regional commission for Community Health Workers is required.

Responsibilities

III. PRIMARY RESPONSIBILITIES:

  1. Provides direct supervision of Community Health Workers (CHWs) including hiring, orientation, training, and support ongoing professional development of team.
  2. Manages tasks for CHWs including monitoring, distribution, and evaluation of completion.
  3. Consults with Community Health Director to address areas of concern, reporting incidents, policy compliance, risk management, and overall progress of CHWs competencies.
  4. Monitors documentation, of CHW Team, to ensure data collection meets requirements for health center standards and grant deliverables of funding sources.
  5. Assists Community Health Director with budget development and evaluation.
  6. Responsible for oversight of payroll for CHWs, including timecards through ADP TotalSource portal.
  7. Assists Community Health Director with reporting through evaluation of data collection and analysis.
  8. Assists Community Health Director with conducting performance evaluations for CHWs.
  9. Serves as a leader for modeling customer service, screening patients’ social determinants of health, and resource navigation.
  10. Collaborates with care teams to create solutions for patients to overcome barriers to achieving healthcare goals.
    11.. Provides public health services for patients and their families in accordance with Center policies and protocols. Makes home visits to patients based on referrals from providers, nurse educators, and behavioral health staff.
  11. Consults with staff members regarding the comprehensive healthcare of patients and families.
  12. Acts as an advocate for the rights of culturally diverse patients and their families in the health care system and community.
  13. Utilizes care plan to document progress of individuals and their families on meeting their social needs, records relevant data for outcome reporting in medical record.
  14. Assists with the implementation of the Center’s community health promotion activities in cooperation with other team members and community.
  15. Establishes cooperative referral relationships with organizations outside of the Center to provide needed resources to patients and their families.
  16. Participates in trauma-informed care supervision training and ongoing supervision professional development.

IV. PERIODIC DUTIES:

  1. Contributes to Health Center community health activities outside of regular job responsibilities.
  2. Attends and participates in department meetings and community activities as assigned.
  3. Performs other duties as assigned.
