We’re searching for a Community Health Worker, someone who’s ready to be part of the best ranked children’s hospital in Texas, and among the best in the nation. In this position, you will provide outreach activities to patients and their families for locating and utilizing medical homes. Collects, completes and  inputs accurate data related to the outreach activities and family data.   Think you’ve got what it takes? Job Duties & Responsibilities Screens families for access to medical home by following program criteria. Provides one on one help to the family in locating a medical home. Educates families about medical homes and their appropriate use. Provides screening questionnaires to determine resources needed by family. Collaborates with the Medical Director and/or Mobile Clinic Program Manager proactively to ensure that appropriate forms are used, and correct process is followed for CHIP and Medicaid applications. Also ensures that appropriate information regarding low-cost health care alternatives is provided to families. Reviews with the family CHIP and Medicaid eligibility/renewal for eligible children. Reviews of low-cost health care alternatives for ineligible children. Communicates requirements in a clear and understandable way to patients and families. Maintains professional conduct when interacting with patients, families, and members of the patient’s health care team. Performs other tasks necessary for coordination and performance of outreach activities, including communicating the progress of outreach activities, identifying problem areas, and making suggestions for improvement. Reviews proposed outreach activities and other mobile clinic related programs and provides feedback to the Medical Director and/or Mobile Clinic Program Manager. Assists with the planning and development of new outreach activities as necessary Collects and completes data collection and queries related to outreach and other mobile clinic activities by obtaining information within specified time frames and submitting the information as required. Participates in meetings and planning sessions with mobile clinic team.   Skills & Requirements Required H.S.
