Community Health Worker (CHW) Full-Time | Community-Based Role Do you have a heart for helping others and a strong connection to your community? We’re looking for a Community Health Worker (CHW) to join our care coordination team and support patients where it matters most — in their homes and communities. As a CHW, you’ll work directly with high-risk patients, providing education, encouragement, and hands-on support to help them manage chronic conditions, access resources, and stay connected to their healthcare team. What You’ll Do * Visit patients in their homes and community settings to provide ongoing support * Help patients understand and follow care and self-management plans * Provide social, emotional, and practical support as patient needs change * Serve as a liaison between patients, providers, and community resources * Assist with care coordination, health education, and access to services What We’re Looking For * A trusted, compassionate member of the community * Strong communication and interpersonal skills * Able to understand written materials and document information clearly * Ability to show empathy and build meaningful relationships * Knowledge of local community resources * Valid, unrestricted driver’s license in the state of employment * Position will require frequent travel.
