Major Responsibilities: The service provider will be responsible for the following tasks in coordination and collaboration with the program team,  Conduct community needs and baseline assessment in the targeted locations in collaboration with the program team  Maintain and establish relationships with community leaders and stakeholders to support health activities.  Provide support to both the protection and health team during the program activities.  Deliver health education sessions using culturally appropriate language and materials and distribute health promotion materials, and facilitate community understanding of key health messages.  Facilities referrals and link individuals to appropriate health and social services by providing accurate information and documentation.  Collaborate with medical and protection teams during outreach activities, providing logistical and translation support as needed.  Maintain confidentiality and uphold ethical standards in all interactions.  Support data collection and accurately tracking health activities  Support the development and implementation of surveys (e.g., KAP, satisfaction) in collaboration with the health team.  Participate in planning meetings and training sessions to enhance service delivery.  Contribute to the identification of gaps in community health services and propose actionable solutions.  Provide a weekly report and submit the data on a weekly bases to the system.
