POSITION QUALIFICATIONS

The successful candidate will have a degree or diploma in a Human Services field with 2 to 3 years of direct experience in working in a supervisory capacity in a residential setting.

  • An open, accepting and non-judgmental attitude with an ability to build rapport with clients and staff
  • Experience working with people with complex life circumstances associated with substance-related disorders, mental health issues, mobility issues and homelessness
  • Basic understanding of harm reduction principles based on dignity, hope and healing as individuals move forward towards improved health and well-being
  • Demonstrates understanding of the addictions field
  • Knowledge of referrals resource
  • Valid Driver’s license
  • Able to use their own vehicle and purchase a minimum of $2 million in liability coverage
  • Current Vulnerable Sector Police Information Check
  • First Aid, CPR, AED Certification

Responsibilities

The Community Engagement Case Manager, under the direction of the Program Manager, is responsible for conducting ongoing needs assessments and developing personalized treatment plans to support individuals in their recovery journey. This role involves implementing tailored interventions, providing direct support, and facilitating access to community resources. The Case Manager conducts intake assessments to understand clients’ needs, goals, and barriers to recovery, ensuring that each individual receives the appropriate referrals, advocacy, and guidance. By fostering community engagement, the Case Manager helps clients establish meaningful daily activities, build supportive networks, and enhance their overall well-being.
The Community Engagement Case Manager will play a key role in recognizing and reinforcing positive behavior changes, while identifying and addressing barriers that may hinder clients’ ability to build and maintain relationships with community resources. A crucial aspect of this role is supporting clients in developing their recovery capital—the personal, social, and community resources that contribute to sustained recovery and overall well-being.
The Community Engagement Case Manager will have a strong understanding of Recovery and Harm Reduction practices and the willingness and ability to work within these frameworks.
