Berkshire Healthcare NHS Foundation Trust is looking for a Community Learning Disability Nurse , Band 6, 37.5 hours per week , permanent contract , to work in our Community Team for People with Learning Disabilities in the Wokingham locality .
We are looking for a learning disability nurse who will join our multi-disciplinary team and help to make a difference to the quality of life for people with learning disabilities. This is an exciting opportunity to work with a wide range of people with many diverse and complex needs. This is a great opportunity to work in the community providing you with a unique opportunity to develop and learn within the learning disability service.

You will be expected to do some of your visits remotely & face-to-face, working in a multi-disciplinary team approach, including working with private care providers and families. The ability to educate and influence is therefore essential.

To manage a caseload
To contribute to comprehensive specialist nursing assessment
To promote and facilitate access to primary and secondary healthcare for individuals with a learning disability including health checks and health action plans
To provide specialist advice and therapeutic interventions to parents and carers of people with a learning
To provide and facilitate health education and promotion to include both individual and group work
