Description About the Role Corus Health is seeking a Community Liaison to join our Albuquerque-based team, supporting both our Home Health and Hospice service lines. This is a high-impact role for a relationship-builder who thrives in the post-acute care space and wants to help shape the future of care in our community. Key Responsibilities Promote awareness and appropriate utilization of Corus Health’s home health and hospice services. Build and maintain referral relationships with hospitals, SNFs, ALFs, physician practices, and other post-acute partners. Coordinate referrals and provide timely follow-up to ensure smooth transitions of care. Educate healthcare professionals and the public about the hospice Medicare benefit and in-home services. Collaborate closely with clinical and operational teams to ensure alignment and excellence in service delivery. Support the development of marketing strategies to achieve admission and census goals. Represent Corus Health professionally at community events, in-service trainings, and outreach initiatives. Analyze referral trends and provide strategic feedback to leadership. Maintain compliance with HIPAA and all relevant regulations. Requirements What We’re Looking For Deep connection to the local post-acute healthcare community — you know the players and how to engage them. Proven success in a healthcare sales, outreach, or liaison role (preferably in home health, hospice, or senior care). Strong communication skills and the ability to speak confidently with clinical teams, referral sources, and families. A self-starter with strategic instincts and the drive to grow in leadership. Ability to work independently while collaborating across departments. Bachelor's degree preferred; clinical background a plus. Valid driver’s license and reliable transportation required.
