In this highly visible, relationship-focused role, you'll build strong referral partnerships, grow service awareness, and support access to home health, hospice, and advanced illness services throughout your assigned territory. What You'll Do Grow Community Partnerships Develop and strengthen relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral partners. Conduct daily in-person outreach and educational visits throughout your assigned territory. Identify new business opportunities and cultivate prospective referral sources. Represent Granite VNA at networking events, community programs, and professional meetings. Manage and Expand Referral Accounts Serve as the primary contact for a portfolio of referral accounts. Develop account-specific growth and retention strategies. Monitor referral trends and identify opportunities to increase market share. Ensure exceptional customer service through responsive communication and follow-through. Collaborate for Success Partner with clinical, intake, and operational teams to support seamless transitions of care. Share market insights, referral trends, and competitive intelligence with leadership. Support organizational growth initiatives and strategic goals. Requirements Education & Experience Bachelor’s degree in marketing, business, social work or related field preferred. Minimum of 2 years of healthcare sales, hospice, home health, or related business development experience preferred. Experience managing provider relationships and referral accounts strongly preferred. Skills & Competencies Strong interpersonal, communication, and presentation skills. Proven ability to build and sustain professional relationships. Demonstrated sales, networking, and account management abilities. Knowledge of home health, hospice, and healthcare referral processes preferred. Ability to work independently while managing multiple priorities. Strong organizational and time management skills. Proficiency with CRM systems and Microsoft Office applications. Physical & Travel Requirements Frequent local travel within assigned territory required. Valid driver’s license and reliable transportation required. Ability to spend significant time in the field conducting in-person visits.
