Description About the Role: Touchstone Health is looking for a compassionate and connected Community Liaison to grow awareness and access to our Home Health and Hospice services in San Antonio. This is a field-based role, perfect for someone with strong healthcare marketing experience who thrives on building lasting referral relationships and improving care transitions. Key Responsibilities: Promote the benefits of home health and hospice services to referral partners and the wider community Build and maintain relationships with hospitals, skilled nursing facilities, and physician offices Coordinate referrals and ensure timely, seamless follow-up Identify education opportunities and assist with in-service presentations Collaborate closely with clinical staff to ensure continuity of care Participate in territory planning and help achieve admission goals Analyze referral trends and identify new opportunities for growth Represent Touchstone Health and CareM’s mission with professionalism and heart Maintain HIPAA compliance and uphold ethical marketing practices Requirements What We’re Looking For: Proven success in healthcare marketing, preferably in home health or hospice Strong relationship-building skills and a people-first mindset Familiarity with SNF-to-Home transitions is a plus Ability to work independently in a field-based role Highly organized with excellent follow-through Clear, compassionate communicator Motivated by mission, integrity, and community connection Bachelor’s degree preferred Why Join Us: At Touchstone Health, we do more than deliver care — we create connection, trust, and comfort during life’s most important transitions.
