Description Harmony Hospice, LLC is seeking a compassionate, relationship-driven Community Liaison to represent our mission throughout the community and expand awareness of hospice services. This role focuses on building trusted partnerships with healthcare providers, facilities, and community organizations while educating families and professionals on the benefits and value of hospice care. Requirements Community Engagement & Outreach Build and maintain strong relationships with referral sources including: Skilled nursing facilities, Assisted living communities, Hospitals, Physician offices, and Home health agencies Represent Harmony Hospice at community events, health fairs, and professional networking opportunities Increase community awareness and understanding of hospice philosophy and services Referral Development Identify and develop new referral partnerships Maintain consistent contact with existing partners to strengthen long-term relationships Collaborate with clinical leadership to support smooth referral transitions and admissions Provide education on hospice eligibility and appropriate timing of referrals Education & Advocacy Educate families, providers, and community partners on: Hospice benefits and services Medicare hospice eligibility Goals-of-care conversations Serve as a trusted resource for end-of-life education in the community Support in-services and educational presentations for facilities and provider groups Internal Collaboration Work closely with the Executive Director and clinical team to align growth strategy Provide market insights and feedback on community needs and trends Participate in team meetings and contribute to strategic growth initiatives Qualifications At least 1 year of experience in marketing, nursing or social work interacting with health agencies/professionals Must be organized and detail oriented Must be able to communicate effectively orally and in writing Must be licensed driver with an insured automobile in good working order
