Description Community Vision is looking for a Community Living Coordinator to join our team. The Community Living Coordinator supervises direct support staff and works one-on-one with people in the Community Living program; providing direct support, coordinating staff schedules, and facilitating various activities to ensure the individuals receiving our services have opportunities at home and in the community. The ideal candidate for this role is someone who is has a trauma-informed approach to supporting people, is highly organized, adaptable, and has the ability to quickly establish healthy connections with the individuals receiving our services and their natural supports (i.e., family members, neighbors, friends, etc.). Key Job Functions: Program Management (50%) - The Community Living Coordinator completes administrative and managerial duties including supporting program development, coaching and supervising direct support staff, and completing accurate documentation. Direct Support (30%) - The Community Living Coordinator provides individualized direct support, which may include, but is not limited to: financial management, medical management, grocery shopping, community activities, household maintenance, and creating and maintaining relationships through social skill development. Plan Coordination (20%) - The Community Living Coordinator works with the individual and their support team, including families, in identifying and monitoring specific activities to achieve goals in their Individual Support Plan, according to the Service Agreement. Requirements At least 3 years of paid experience in developmental disabilities field, or in human services. At least 1 year of paid experience managing others; preferably with experience coordinating schedules Frequent driving is required for this position. Candidates must be available/able to reliably travel to various locations within a 20 mile radius of our SE Portland Office. Auto liability insurance must be carried through the duration of employment and a clean driving record with a valid Driver’s license is required. Must be able to push, pull, and lift up to 50 lbs. Must be able to obtain approved Criminal History Check through the Oregon Department of Human Services Background Check Unit prior to working independently, without direct supervision. Must not be on the Office of Inspector General's (OIG) List of Excluded Individuals Must maintain effective CPR/First Aid Certification and OIS Certification (on-site, paid training provided by CV).
