As Community Medication Coordinator with VOAMASS, you’ll have a dynamic and critical role working closely with Registered Nurses, PMHNPs, the Medical Director, integrated care teams, VOAMASS programs, and external providers to support and enhance the whole health of the individuals we serve. As Community Medication Coordinator you will review medical information, collect vital signs, complete preventive care screenings, provide patient education, manage prior authorizations, and perform medication reconciliations. Some of your critical responsibilities: Provide assessments and physical examinations to individuals with mental health, substance use, dual diagnosis, and co-occurring disorders. Collect vital signs, complete preventative care screenings, complete routine procedures, and report on person served symptoms and collaborate with PMHNP’s. Develop, implement, and revise integrated care plans that reflect person served diagnosis and outlined standards of care. Will coordinate closely with PMHNP’s and Medical Director, PCP’s, and external medical providers to enhance the whole health of the people that we serve. Provide support to PMHNP’s prescribing practice for prescription of medication, completes insurance prior authorization and medication reconciliation in conjunction with PMHNP’s. Serve as part of the 24/7 mobile crisis team to ensure that persons served are being provided assessments, preventative healthcare screenings with a no four walls approach. May intervene in emergency or crisis situations according to 24/7 mobile crisis protocol. Share on-call duties with lead registered nurse. Serve as a key member of the Integrated Services Care Team, embedded within our programs and multidisciplinary review processes to enhance the experience of persons served and ensure agency compliance with state regulations and professional medical standards. Provides clinical consultation, guidance, and training to program and clinical staff as part of the integrated services team. Maintain records according to VOAMASS Policies and Procedures, completing all documentation and billing in a timely manner. The Community Medication Coordinator role is about making a difference in the lives of the people we serve. You’ll bring the following qualifications: An Associate’s or Bachelor’s Degree in Nursing is required. Candidates must be actively working toward obtaining their Registered Nursing (RN) license and must obtain RN licensure within one year of hire. Prior experience working with individuals with mental health conditions, substance use disorders, dual diagnoses, and co-occurring disorders is essential. Candidates must also be willing to work across various VOAMASS locations. Strong patient-centered care skills, excellent communication abilities, and the capacity to work collaboratively within a multidisciplinary team are required.
