An exciting opportunity has arisen to work as a Team Leader for Harrow Community Mental Health Triage and Assessment Team. We are looking for an experienced Mental Health Nurse to join the team to work in the community with Adults with mental health needs and support the team with the day to day running of a busy triage mental health service
The role is suitable for nurses who have experience in working in the community, with the multidisciplinary team, individuals with mental health difficulties and for someone seeking to develop and apply their leadership and management skills.
Harrow Triage and Assessment Team, based in Harrow and Wealdstone is looking for a passionate RMN and Team Leader who is looking to make a difference whilst working in an environment that promotes staff development and training opportunities . CNWL is passionate about developing its own staff , driving them to go on to become more established practitioners and this post is no different.
The Harrow CMHH is an exciting area to work in as it is at the forefront of mental health transformation in line with the five year forward plan and Harrow services are pioneers in the Trauma Informed Approaches. To be considered for this post you will need to be a registered mental health nurse and be keen to develop experience of working in multi-disciplinary teams who care for adults within mental health secondary care sector.
The post holder will work as a Clinical Team Leader in the team providing brief clinical and social care intervention for service users.
To Triage referrals and co-ordinate duty and assessments,
Be responsible for the supervision of staff & their caseload.
Facilitate appropriate clinical practice of the team including RMNs, care navigators, social workers and the administrative staff.
Draw up the monthly rotas
Develop and maintain close professional relationships with GPs, multi-agencies and other healthcare professionals.
Ensure staff practice in a safe, appropriate and professional way with service users and their carers.
Attend professional practice forums
Organise and investigate Safeguarding alerts in line with local Safeguarding policies.
Chair clinical and team meetings as appropriate.
Provide senior support to the Team Manager
To assist the team to be compliant with KPIs and monitor electronic records

To receive referrals from SPA and GPs, triage, screen and undertake comprehensive psycho-social mental health assessments to establish care needs.
To screen non-urgent GP Adult mental health referrals, identifying patients who are suitable for management in primary care according to agreed thresholds and with any adjustments to reflect MH resource available at individual practices
To act as a contact point and provide a link/liaison role to GP practices with regard to patients with more complex mental health problems and/or co-morbidities
To facilitate onward referral and rapid access to mental health services and other agencies in line with agreed thresholds and shared protocols and criteria, and/or to provide support/advise referring GP on effective management
To promote and support self-care, recognising the important role of individuals, their families and carers as co-producers of health.
To identify vulnerable adults and provide effective programmes of support that protect and promote their health and wellbeing, intervening appropriately when a person’s safety or welfare is at risk.
To ensure that CNWL Trust and LA protocols are adhered to by team members.  To cover for the absences of senior colleagues as appropriate and as required by service needs.
To contribute to the implementation of the Clinical Governance agenda including Target Setting for Teams, Clinical Audit.
Participate as a key member of the Borough Service to ensure collaborative and innovative practice
