We are seeking an enthusiastic registered nurse to join our clinical team.

Responsibilities

ABOUT THE ROLE

Community Mental Health Services provide a range of services designed to help support the recovery and help consumer stay well in the community. The team provide specialist services for young people, children and families, adults, and older people. Community mental health services include a specialist Aboriginal Mental Health Team, bilingual mental health workers and Peer Support Workers. Our main sites are at Canterbury, Croydon, Camperdown, Marrickville, and Redfern Community Health Services. The role will be based at Croydon Health Centre.
The Community Mental Health/Core Team model of care states:
The “Community Mental Health Team” (Core Team) is the mainstay of the community mental health system. Core Teams and provide a range of clinical services including intake, assessment, consultation, treatment ,and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care, and short term care coordination.

Consumers under the care of Core Teams will either have:

  • Short term interventions and may be discharged or referred back to their GPs or other treatment providers after a period of weeks or months.
  • A period of treatment as recommended by evidence based practices.
  • Ongoing care and monitoring for prolonged periods.
