We are looking to recruit a highly skilled, motivated and passionate Band 6 Community Mental Health Nurse to join our Adult Community Mental Health Team in the South & West area of Bradford, providing care to service users in a large geographical area, covering urban and rural communities.
The post holder will be expected to work without direct supervision and be able to organise and prioritise their workload. Whilst reporting to the Team Leader, they will be capable of making autonomous clinical decisions affecting the mental health care of clients, based of their own and others’ assessments and observations and requiring analysis of a complex range of information and options.

To manage a complex caseload of clients in the community and to work towards ensuring the safety and wellbeing of all service users, their families, and carers.
To work in partnership with service users, families, and carers to achieve identified goals and to support recovery and reintegration by adopting a fully holistic approach to needs.
To administer medications safely and monitor their efficacy.
To accurately record clinical information into our systems including risk assessments, care plans, care act assessments, crisis and contingency plans.
To work as part of our duty service triaging calls and signposting to address identified needs.
To work within a fully integrated and jointly managed Community Mental Health Team alongside Local Authority social care staff and managers and to liaise and feedback on all aspects of service user care.
To assess any social care needs of your caseload and look to source and arrange these in line with the operational policies.
You will also be mentoring and supervising both preceptee nurses and student nurses, enhancing their placement experience and supporting their learning and development.
