JOB OVERVIEW

Ellesmere Port Community Mental Health Service are seeking a number of enthusiastic community mental health nurses to join our busy team.
Do you have a passion for supporting people within their own home to live as independently as possible? You will be joining a busy community mental health team who provides support to adults with a serious mental health condition such as Bi Polar Affective Disorder, Schizophrenia, Severe Depression to maintain their quality of life and remain well in the community .
We are ideally looking for a practitioner with experience of both functional and organic mental ill health. However, we would consider a nurse with experience of one of these areas as training and support will be given to develop your skills and experience.

Responsibilities

MAIN DUTIES OF THE JOB

The post holder will work autonomously within a multi-disciplinary framework, undertaking assessments and coordinating the care of patients with both functional and organic mental illness.
The post holder will maintain a small caseload which will involve ongoing assessment: Formulation of care planning, risk assessments. Completing reviews and offering interventions to help improve wellbeing/outcomes for the individual and their families, signposting to other agencies as necessary. The post holder will be expected to be clinic based for majority of their role and be allocated duty practitioner as per rota Excellent communication, report writing, and assessment skills are essential for this post. Applicants should have sound working knowledge of mental health disorders and the Mental Health Act.
We are a friendly and supportive team upholding the core values of CWP. We offer a thorough induction period, which includes shadowing established team members alongside ongoing supervision and support . Supervision and training are mandatory with wider training / personal development readily available.
Access to transport for work purposes is essential.
