Description The Caseworker works in collaboration and conjunction with youth, families, and the school system in contracted partnership with ECSD to insure consistent and regular school attendance and performance for all identified youth on their caseload. The Caseworker utilizes their professional knowledge, skills, and experience to identify, assess, and intervene in areas of individual or family functioning that contribute to and inhibit school attendance, academic success, or overall school citizenship. The Caseworker adopts a “whole system” approach to treatment and as such must negotiate within multiple systems and locales (i.e., schools, community partners etc.) Primary Job Functions 1. Provide school-based support and intervention companied with family support and planning to improve and encourage, increased school attendance, decreased discipline and problematic behaviors by educating and utilizing safety planning and coping skills and promoting academic success using goal planning and incentive-based programming. To provide support to school personnel, parents, families, and students as identified based upon their assigned caseload. · Performs all work tasks in a safe manner. · Is guided by organizational values and trauma informed practice in all interactions. · Demonstrates understanding of the four dimensions of safety and can identify gaps. · Treats all children, families, staff and ancillary providers with respect and courtesy. · Demonstrates and models sensitivity to the cultural background of children, families, and co-workers. · Assures that job-related activities are following Glove House policies and procedures, ECSD standards, State and Federal regulations, and relevant professional association, ethical standards, accreditation standards, and the law. · Provides casework contact to focus on goal attainment and safety. Mentors should have contact with students daily. · Provides individual, family mentoring services to a caseload of 10-12 youth/families at any given time. The stated purpose of said efforts is to assist youth in being successful in the school environment, both academically and socially. · Maintains daily contact with identified youth and 2x weekly contact with families and school personnel via school meetings, etc. · In conjunction with clients and school personnel, develops individual and family treatment plans and facilitates ongoing measurement, evaluation, and modification of goals where appropriate. · Prepares required reports concerning clients/families/systems including treatment goals, progress, barriers, and contacts. · Completes all program documentation, both written and electronic, as required by program rules and regulations. · Assures that rules, recommendations, and accountability requirements of appropriate regulatory bodies-i.e., NYS Department of Education, Elmira City School District, etc. are adhered to in relation to clients served. · Acts as a liaison and facilitator, i.e., advocacy, referral source between client and ancillary service agencies, etc. · Is instrumental in the provision or facilitation of needed supportive services. · Maintains information on community resources, which can be utilized during treatment or to make appropriate referral at the time of discharge. · Participates in the process of transferring clients to other levels of intervention as appropriate. · At the Middle and High School level, encourage students to obtain credits towards graduation. · Is required to drive on agency business as required by job demands. Maintain accurate mileage documentation and submit in required agency time frames. · Is required to complete and maintain all mandated training within required agency time frames and within pre-established passing grade levels. · Must maintain timely compliance with all established agency standards and provide support documents as necessary (physical/TB, driver’s license, auto insurance). · As an employee of Glove House, Inc., one of your primary responsibilities is to protect the privacy of our clients. Among these regulations are the Privacy Rule and Security Rule issued by the U.S. Department of Health and Human Services (HHS) under the Health Insurance Portability and Accountability Act of 1996 (HIPPA). · Conduct community outreach as needed. · May monitor interns and/or volunteers. 2. Develop and maintain case records and program documentation, such as progress notes, goals, family meetings, summaries, databases, attendance and reports according to contract and Glove House standards. · Generate and maintain necessary reports and paperwork (i.e., Quality Assurance and program reports). · Assures all documentation is completed in a timely fashion i.e., youth notes, Class I and II, etc. · Will actively participate as a member of the School Based Services team. · Consistently attends staff meetings and provides leadership and partnership during team/staff meetings. · Assures that program staff are up to date with any concerns or needs of your case load. · Participates in Glove House communications and committees. · Responsible to assist in the research and development of best practices and new initiatives to meet the changing needs of children and families in the community. · Must maintain timely compliance with all established Glove House standards and provide support documents as necessary (physical/TB, driver license, auto insurance). · As an employee of Glove House, Inc., one of your primary responsibilities is to protect the privacy of our clients. Job Qualifications Education, Licenses and Certifications All degrees must be from an accredited university. · Related Bachelor’s degree preferred; Bachelor’s degree required. Must have a valid driver’s license and driving record in accordance with agency standards. Experience · Minimum 2+ years’ experience working with children and families in a mental health, criminal justice or education setting capacity. · Knowledge, Skills, and Abilities Knowledge of · Community resources, casework, NY State education requirements, Abilities to: · Perform under strong demands in fast-paced, diverse environment. · Handle confidential information appropriately. · Perform at a high level of autonomy, with minimal supervision. · Ability to read and interpret documents such as contracts, regulations, training manuals, and procedural manuals. · Ability to write routine reports and correspondence. Utilizing computer and standard Microsoft Office, learn new computer programming (ie Myevolve, Paycom, et al.) · Ability to speak effectively before groups of customers or employees of organization. · Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. · Ability to deal with problems involving several concrete variables in standardized situations. · Work on multiple tasks, while maintaining a high attention to detail, accuracy, and quality · Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. · Commit to effectively and efficiently providing high levels of customer satisfaction with positive service delivery results. · Demonstrate the behaviors of the Glove House Values · Interact effectively with individuals/families from diverse backgrounds. · Set appropriate limits and boundaries with clients; act decisively to protect clients, when necessary. · Assess and apply proper intervention strategies. · Identify and understand environmental stressors. · Recognize importance of social, economic, environmental factors in the development and resolution of personal/family problems. · Be sensitive to service population’s cultural/socioeconomic characteristics. · Commit to effectively and efficiently providing high levels of customer satisfaction with positive service delivery results. Schedule Expectations The incumbent will work an agreed upon standard scheduled week, between the hours of 7:30 am and 4:30 pm Monday- Friday. Travel will be as needed, to conduct Glove House business-related activities. Generally, duties of this position require at least 80% time at work/face-to-face time with clients, co-workers, and supervisor. Up to 20% may be approved to work from home as approved by supervisor.
