Description

We are delighted to be able to advertise this position to become part of our integrated, multidisciplinary team covering North East Hampshire.
You will be working in the community, treating patients in their own homes across Fleet, Farnborough, Aldershot and Yateley.
You will be part of the nursing team, which forms part of our Enhanced Recovery and Support at Home team, along with OTs, Physios, APs, Rehab Support Workers and admin.
We are a rehab service, supporting patients at home either to prevent admission to hospital or on their discharge home. Referrals are usually from the wards or from the GP, Paramedic Practitioners, members of the ICT etc. In addition to holistic assessments, primary nursing needs include catheter care, wound care and medication management.

THE NURSING ASSOCIATE WILL:

Develop understanding of all elements of the nursing process and be able to assist the registered nurse in the on-going assessment, planning, management and evaluation of care
Provide and monitor care
Support individuals with all aspects of care including personal care, daily living, providing person centred care, and promoting health and independence through awareness raising and care navigation to other professionals as appropriate
Perform and record clinical observations including blood pressure, temperature, respirations, pulse
Be prepared to undertake additional skills once competent including but not exhaustive; cannulation, venepuncture, ECGs as per training and competency and as relevant to the role
Assist with fluid and nutritional/dietary needs of patients and accurately record nutritional and fluid intake including completion of menu cards and recording intake and output of fluids and solids.
