Responsibilities
  • Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
  • Working with patients (both in-person and virtually) to provide exceptional nursing services
  • Providing services in a variety of settings which may include in-home, clinic, retirement home or school
  • Communicating and liaising with patients, family members and others within the interdisciplinary care team
  • Performing other clinical duties as require
