  • Unrestricted Registration with AHPRA as a Registered Nurse
  • Experience within a community setting
  • Sound clinical knowledge and skills
  • Experience working with clients who have chronic and complex health needs
  • Experience working with clients from diverse cultural and socio-economic backgrounds
  • Excellent verbal and written communication skills
  • Excellent time-management skills and attention to detail
  • National Police Check Clearance
  • Working with Children Check and NDIS worker screening
  • Current CPR Certificate

Responsibilities
  • Conduct comprehensive clinical assessments and develop clinically appropriate care plans
  • Provide a range of nursing services to clients within the community, including but not limited to Medication Management, Wound Care, Diabetes Management, Catheter Care, Stomal Therapy and Cognitive Assessments
  • Work holistically with other healthcare professionals to provide a multidisciplinary approach to client care
  • Educate clients and their families to optimise the client’s ability to live safely within the community
  • Work collaboratively with Case Managers and NDIS Support Coordinators to provide positive client outcomes
  • Work autonomously to prioritise and complete tasks and complete all required documentation to a quality standar
