As the Community Nurse (CN) liaison you will form part of the Complex Planning and Pathways team and will be responsible for the identification and facilitation of patients who will be discharged from bed-based services to the Community Nursing program with appropriate supports in place. As the CN liaison officer will also provide monitoring of current Community Nursing patients in bed-based services and ensure adequate and timely discharge planning in preparation for transition to the community setting.
